  • Parent must sign and date the form.
  • Submit one (1) form per child registering.
  • You can register your child for more than one session using one form!
  • Make checks payable to: Half Hollow Hills Gymnastic Workshop
  • Mail completed and signed form and check for the total session fees to:

  • We recommend your child bring a drink and a snack!
  • NO jewelry - Hair MUST be UP...!!!
  • You must fill in, date and sign the Insurance Waiver form (below)!

AM Sessions are: - 9:00am - 12:00noon
PM Sessions are: - 1:00pm - 4:00pm

I am fully aware and appreciate the risk of injury associated with participating in gymnastics. I agree that the Half Hollow Hills Gymnastic Workshop Inc., Half Hollow Hills Central School District employees, officers, and directors shall not be liable for any injury or related medical expenses as a result of my child's participation in gymnastics. Half Hollow Hills School District is not affiliated with Half Hollow Hills Gymnastics Workshop.
